Job summary

Milestone, Inc. in Loves Park, IL seeks a Lead QIDP to develop, monitor, and document individual served programming aimed at maximizing growth and independence. You will train direct care staff and serve as the residential QIDP for assigned individuals, coordinating with the Interdisciplinary Team.

Requires a 4 Year Degree in human services and 1+ year in developmental disabilities, a valid driver's license, and crisis intervention abilities. Travel between sites is expected.

Qualifications

• Bachelor's degree in human services field and at least 1 year of experience in developmental disabilities.
• Must be able to intervene physically in a behavioral or health related crisis.
• Must have a valid driver's license and be able to travel between sites.
• Must meet all requirements to qualify for QIDP status on the QIDP registry.
• Minimum age of 21.

Responsibilities

• Develops, monitors, and documents individual served programming directed toward growth and development.
• Functions as the residential QIDP for assigned individuals served.
• Trains direct care staff in implementation of individual served training programs.
• Leads the Interdisciplinary Team in developing programs to maximize independent functioning.
• Prepares monthly summaries and quarterly reviews for assigned group homes.
• Coordinates assessments and annual meetings for individual served's plans.
