ChenMed in Florida is seeking a Primary Care Physician to provide comprehensive care for an aging patient panel while leading a collaborative care team. The role emphasizes accountability for outcomes, patient-centered coaching, and cost-effective practice within a value-based care model.

You will work with hospitalists, APPs, and clinic staff, maintain licensure, and uphold quality standards. The position offers competitive compensation with a broad benefits package and opportunities for

Qualifications

MD or DO in Internal Medicine, Family Medicine, or Geriatrics is required.
Must be able to obtain a State Medical License for the working state(s).
Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred; Board Eligibility required.
Current DEA number for controlled substances and BLS certification within 90 days.

Responsabilités

Provide care for older adult patients with acute and chronic illnesses.
Assume accountability for patient outcomes and coordinate with specialists when needed.
Collaborate with hospitalists and engage with hospital settings when patients are admitted.
Lead the care team and coordinate care across office, hospital, SNF, and homebound settings.
Engage in recruiting patients for the center and assisting in provider recruitment when needed.
Supervise and collaborate with APPs and remain accountable for their actions.
