Job summary

Keystone Distinction Group is seeking an Advanced Practice Provider (NP/PA) for a full-time, on-site role in Apple Valley, California. The position offers a competitive compensation package and a Monday–Friday schedule with standard business hours.

The successful candidate will deliver high-quality primary care, manage patient panels, and coordinate holistic treatment plans under the Medical Director. Strong communication and teamwork are essential in this collaborative environment.

Qualifications

• Active, unrestricted CA NP or PA license is required.
• Active, unrestricted DEA registration is required.
• Minimum of 2 years of dedicated practice as an Advanced Practice Provider in primary care.
• Strong communication, critical thinking and clinical prioritization skills.

Responsibilities

• Perform comprehensive physical exams, take medical histories and manage treatment plans for primary care patients.
• Order tests, interpret results and refer to specialists to support continuity of care.
• Lead end-of-life and hospice discussions when appropriate.
• Collaborate across clinical departments and participate in quality improvement initiatives.
• Maintain high clinical standards and understand HCC coding requirements.
