Qualifications

• Active, unrestricted medical license in the state of employment.
• Experience leading interdisciplinary teams.
• Experience in clinic, community-based organization, or inpatient unit.
• Excellent interpersonal communication skills.
• Strong problem solving, project management and organization skills.
• Independent self-starter and critical thinker focused on population health.

Responsibilities

• Provide Primary Care services to a panel of members.
• Lead an interdisciplinary care team and coach providers.
• Collaborate with Medical Director on clinical leadership and guidance.
• Serve on leadership teams and partner with organizations to enhance services.
