PACS in Ojai, CA is seeking a Staff Physical Therapist to evaluate and treat residents, develop treatment plans, and collaborate with physicians and families. You will supervise PT assistants and aides, ensure accurate documentation, and participate in care conferences to support rehabilitation goals. Must hold a California PT license and a degree in Physical Therapy, with advanced degrees preferred.
Qualifications
Bachelor’s degree in Physical Therapy; Master’s or Doctorate preferred.
Proficient with Net Health and Point Click Care.
Licensed to practice Physical Therapy in the state.
CPR certification as required by licensure.
Responsibilities
Evaluate residents to determine PT needs, functional limitations, and rehabilitation potential.
Develop, implement, and update individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
Supervise PT Assistants and aides in resident care.
Communicate regularly with physicians, nursing staff, families regarding progress and discharge planning.
Complete timely, accurate documentation in compliance with requirements.
