Qualifications

Responsibilities

• Design and monitor outcomes dashboard and scorecards for assigned programs.
• Monitor and analyze program outcomes; propose and implement action plans.
• Provide feedback to senior leadership on key performance outcomes.
• Present data to program stakeholders to improve performance.
• Develop program components and support implementation of assigned programs.
• Educate provider agencies on best practices to improve outcomes.
