Job summary

Ohio State University Physicians (OSUP) seeks a Nurse Practitioner to lead care in our Skilled Nursing and Sub-Acute Rehabilitation unit in Dublin, Ohio. You will manage high-acuity residents transitioning from acute care to rehabilitation, guiding the care team and coordinating with family members.

The role requires an active Ohio APRN license with national certification and experience in hospitalist/geriatrics settings, plus proficiency with PointClickCare or similar EMR systems.

Qualifications

• Active Ohio APRN license and national certification (AANP or ANCC).
• Minimum of 2 years in hospitalist medicine, geriatrics, or sub-acute rehab preferred.
• Proficiency with PointClickCare or similar EMR; autonomous decision-making; cohesive communicator.

Responsibilities

• High-acuity management of residents admitted post-hospital for short-term stay.
• Conduct admission assessments within 24–48 hours to ensure smooth transitions.
• Collaborate with physicians, therapy, nursing, and social work on discharge planning.
