Qualifications

• Master’s degree required.
• PA-C or NP certification required.
• BLS certification required.

Responsibilities

• Physical assessment and ordering medications.
• Follow guideline therapies and educate patients.
• Transition care from hospital to home and coordinate with care team.
• Outreach to patients and coordinate with primary and specialists.
• Collaborate with home care and paramedicine programs.
• Lead the care team during shifts.
