Responsibilities

• Provide direct patient care in Primary Care clinics.
• Triage and prioritize patient needs as routine, urgent, or emergent.
• Collaborate with PACT and other services to plan patient care.
• Lead and mentor LPNs and assist with clinic procedures and equipment.
