Job summary

Rehab Centre is seeking an experienced LPN/LVN to provide direct nursing care and supervise nursing assistants in a Creswell facility. You will ensure high-quality resident care, complete charting, and coordinate with physicians and the nursing team.

The role requires a nursing degree or LPN/LVN program completion, active state licensure, and CPR certification. Strong leadership and communication skills are essential for supervising shifts and maintaining care standards.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Current CPR certification required; PCC knowledge preferred.

Responsibilities

• Direct the day-to-day functions of nursing assistants in accordance with rules and guidelines.
• Oversee charting, documentation, and reporting to ensure accurate resident records.
• Administer medications as ordered and ensure narcotic records are accurate.
• Provide leadership to nursing personnel and supervise shifts to maintain quality care.
