Sullivan Park Care Center Sullivan SNF Healthcare, LLC is seeking an LPN/LVN to provide direct nursing care to residents and to supervise nursing assistants, ensuring adherence to federal, state, and local standards. This role emphasizes leadership, compliance, and high-quality resident care.

You will oversee nursing operations, charting, medication administration, and care plans, while coordinating with the Nurse Supervisor to maintain a safe and supportive environment for residents and staff.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered active license to practice as an LPN/LVN in this state.
• Must be able to read, write, speak, and understand English.
• Ability to read technical procedures and policy manuals; communicate with managers, residents, and families.

Responsibilities

• Direct the day-to-day functions of the nursing assistants and supervise staff.
• Ensure compliance with written policies and procedures.
• Maintain Nursing Service Procedures Manual and discharge plans.
• Complete resident charting and documentation accurately.
• Administer medications as ordered and verify resident identity.
• Participate in quality assurance and facility surveys.
• Provide leadership and plan staffing for shifts.
