Job summary

Palm Valley Healthcare, LLC seeks an experienced LPN/LVN to lead nursing staff and provide direct resident care in accordance with state and federal regulations. You will supervise nursing assistants, ensure policy compliance, and maintain essential documentation in a long-term care setting.

In this role you will manage admissions, transfers, discharges, care plans, and daily census, while coordinating with the Nurse Supervisor to ensure high-quality patient care and regulatory compliance.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Must be able to read, write, speak, and understand English.

Responsibilities

• Direct the day-to-day functions of the nursing assistants in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility.
• Participate in facility surveys (inspections) and maintain the Nursing Service Procedures Manual.
• Admit, transfer and discharge residents as required and maintain daily census reports.
• Complete accident/incident reports and forward to the Director; write resident charge slips and forward to Business Office.
