Job summary

Signature HealthCARE, LLC is seeking a dedicated LPN/LVN at Ridgely Rehab & Wellness Center to provide direct nursing care, monitor residents, and support daily operations.

The role requires an active LPN/LVN license in Tennessee, CPR/BLS certification preferred, and at least one year of clinical nursing experience, preferably in long-term care. You will supervise nursing assistants and coordinate with the care team to ensure safety and quality.

Qualifications

• Active LPN/LVN license in the state of practice (Tennessee).
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) with dignity and safety.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor residents' condition and inform physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete clinical documentation per facility policy.
