Job summary

Autumn Care Management is seeking a licensed Practical Nurse or Registered Nurse to join our team and provide direct resident care according to the care plan. You will ensure documentation accuracy, monitor health changes, and communicate with families and health professionals as needed.

Responsibilities include supervising Resident Assistants, maintaining safety and confidentiality, and participating in ongoing staff education and care planning meetings as directed by the Director of Nursing.

Qualifications

• Must hold a current LPN or RN license.
• Proven nursing care skills required.
• Ability to supervise Resident Assistants and work in a team.

Responsibilities

• Use universal precautions and infection control during resident care and safety measures.
• Prepare and review resident record documentation per facility policy (admission, care plan, transfers, charting for vitals, etc.).
• Notify responsible party and health care professionals about patient needs (rashes, skin changes, etc.).
• Refill medications as needed and perform self-medication evaluations with residents with doctor orders.
• Complete incident reports and communicate findings to the Director of Nursing.
• Maintain confidentiality and professional appearance; participate in in-service education and staff meetings.
• Attend care plan meetings as directed; coordinate with Executive Director/Director of Nursing on staff availability.
• Represent Autumn Care Management positively to families and the community.
