Job summary

Trilogy Health Services, LLC is seeking an Assisted Living Coordinator in Monticello, IN to coordinate nursing care for residents and manage medications and daily living activities. You will work with the DON/ADON to ensure high quality clinical services campus-wide.

Qualifications

• Active, unencumbered LPN license in the state.
• CPR certification is required.
• Previous assisted living and leadership experience preferred.

Responsibilities

• Collaborates with the DON and ADON for campus clinical services.
• Ensures pre-admission screens are completed prior to move-in.
• Directs lifestyle and care needs assessments for residents.
• Assists residents with activities of daily living and medication administration.
• Leads the development of the service plan with residents and families.
• Facilitates Assisted Living Resident Meetings and communicates feedback.
• Works as the lead nurse and participates in on-call rotation.
