Job summary

Trilogy Health Services, LLC in Maumee, OH seeks an Assisted Living Coordinator to oversee resident assessments, medication management, and daily living activities for residents. The role emphasizes teamwork with nursing leadership and a commitment to quality resident care.

You will collaborate with the Director of Nursing, design and communicate care plans with families, assist with ADLs and medications, and participate in on-call rotations to support the campus.

Qualifications

• Active, unencumbered LPN license within the state.
• CPR certification required.
• Previous assisted living and leadership experience preferred.

Responsibilities

• Collaborates with the DON and ADON relating to clinical services across the campus.
• Ensures pre-admission screen is completed prior to move-in.
• Leads service plan development with resident/family and communicates plan to caregivers.
• Assists residents with ADLs and medication administration.
• Facilitates resident meetings and addresses feedback promptly.
• Acts as lead nurse for assisted living residents and participates in on-call rotation.
