• Collaborate with RN Case Managers, physicians, and teams to coordinate post-acute services.
• Educate patients and families on available resources and benefits.
• Identify and link patients to community agencies and social services.
• Document thoroughly in the electronic medical record and support CMS compliance.
• Participate in rounds and care planning conferences with the interdisciplinary team.
