Job summary

Henry Ford Health in West Bloomfield Hospital is seeking a Supervisor of Integrated Case Management to oversee daily case management operations and guide a team of professionals in delivering collaborative, patient-centered care.

You will ensure assessment, planning, coordination, and evaluation of care, promote patient safety and cost-effective outcomes, and participate in policy development and performance improvement with an interdisciplinary team.

Qualifications

• Bachelor's degree in nursing or related professional field (e.g., social work, counseling, health education) or a Master's Degree of Social Work.
• Three (3) years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
• Strong communication and interpersonal skills.
• Strong analytical, problem-solving, and critical thinking skills.
• Registered Nurse (RN) or Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license.

Responsibilities

• Oversee daily case management operations, ensuring the team delivers high-quality, patient-centered care.
• Monitor and guide case management professionals in assessment, planning, facilitation, care coordination, evaluation, and advocacy.
• Participate in the development of policies, procedures, and daily workflow design.
• Maintain and contribute to performance improvement activities to enhance care delivery.
• Foster collaborative relationships with an interdisciplinary care team to achieve optimal patient outcomes.
