Qualifications

• Three years of clinical experience in case management, disease management, health coaching, nursing, or home health care.
• RN or LMSW with a valid, unrestricted Michigan license.
• Strong communication and interpersonal skills.
• Strong analytical, problem-solving, and critical thinking skills.

Responsibilities

• Oversee daily case management operations, ensuring the team delivers high-quality, patient-centered care.
• Monitor and guide case management professionals in assessment, planning, facilitation, care coordination, evaluation, and advocacy.
• Participate in the development of policies, procedures, and daily workflow design.
• Maintain and contribute to performance improvement activities to enhance care delivery.
• Foster collaborative relationships with an interdisciplinary care team to achieve optimal patient outcomes.
