Job summary

EverHomeHealthCare in the Central Puget Sound region seeks an Occupational Therapist to lead the team and provide skilled in-home therapy per physician plans. Strong communication and independent work are essential to ensure optimal client outcomes.

The role requires at least one year of OT experience, active state licensure, and CPR/BLS. Reliable transportation and a valid driver's license are also required for home visits and coordination with care teams.

Qualifications

• Registered by the American Occupational Therapy Association or licensed in the state.
• Minimum of one year of occupational therapy experience.
• Current CPR/BLS certification.
• Reliable transportation, valid driver’s license and proof of auto insurance.

Responsibilities

• Provides skilled occupational therapy in accordance with the physician’s plan of care.
• Coordinate and oversee skilled intermittent home care for clients and educate caregivers.
• Regularly evaluate and re-evaluate client status and adjust the POC as needed.
• Initiate, develop, implement, and revise the client’s plan of care (POC).
• Address all problems identified in the POC and respond to changes in conditions.
• Communicate with other disciplines involved in the case and maintain documentation.
