• Minimum of 2 years of Home Health RN experience.
• Current Illinois RN license.
• Experience with WellSky (Kinnser) EMR preferred.
• Strong organizational and communication skills; able to manage multiple priorities.

Responsibilities

• Care oversight: monitor case management guidelines, provide staff training, and conduct reviews.
• Coordination: be the first line for issue resolution; collaborate with physicians and clinicians on care plans.
• Patient advocacy: act as liaison to ensure patient needs and preferences are respected.
• Documentation: maintain timely, accurate records of patient care and care plans.
