Qualifications

• One year of clinical RN experience.
• Current CPR certification.
• Must be able to reliably commute to multiple patient residences daily.

Responsibilities

• Create, implement, execute, and coordinate the overall plan of care for home health patients.
• Perform initial/comprehensive nursing evaluations and develop care plans according to state guidelines.
• Maintain accurate documentation and clinical notes of patient status and interventions.
• Ensure efficient visit routing and adherence to plan of care from admit to discharge.
