• Must have a reliable driver's license and be comfortable with daily travel.
• Competency in core home health PT scope is necessary.

Responsibilities

• Own the caseload and manage patient therapy plans.
• Complete in-home assessments and provide skilled PT visits.
• Build and execute clear plans of care including safety and mobility.
• Document thoroughly in Homecare Homebase.
• Coordinate with multidisciplinary teams for patient care.
