Job summary

Jobtailor is seeking a licensed Physical Therapist to plan and administer home-based therapy aimed at maximizing patient function. You will assess patients, document findings, and coordinate with physicians to tailor treatment goals.

You will oversee PTAs and Home Health Aides, ensuring accurate care instructions, timely documentation, and clear communication with the care team and families. A team-focused, patient-centered approach is essential.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid state driver's license.
• Personal vehicle liability insurance meeting applicable state minimums or specified higher limits.
• Preferred: One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement rehabilitation programs consistent with the physician's Plan of Treatment and patient-team goals.
• Provide PTAs and Home Health Aides with written care instructions and oversee their performance and documentation.
• Promptly and thoroughly document patient observations, interventions, and evaluations.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family.
• Submit required evaluations, plans, progress notes, and discharge summaries.
