Job summary

Enhabit Home Health & Hospice in Austin, TX is seeking a registered nurse to coordinate and oversee patient care across home health visits, admissions, and recertifications.

You will collaborate with physicians, patients, and caregivers, ensure schedules, maintain regulatory compliance, and support ongoing professional development. The role emphasizes leadership, teamwork, and delivering high-quality home-based care.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license per state requirements, or compact license.
• Experience as a licensed professional may be deferred with VP operations approval.
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification and valid state driver license.
• Automobile liability insurance as required by law.
• Basic demonstrated technology skills, including mobile devices.
• Preferred management experience.
• Preferred Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish communication.
• Dependable transportation and ability to drive in all weather.

Responsibilities

• Coordinate overall home health services delivered to patients per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Ensure scheduling of patients to field clinicians and maintain care coordination.
