VitalCaring Group is hiring a Clinical Care Manager (RN) for our Webster, TX Home Health branch to lead coordination of patient care and guide clinical decisions to ensure high-quality outcomes.

The role requires an active RN license, strong clinical judgment, and the ability to work independently in a fast-paced field-based environment. Home health experience is preferred, with a focus on collaboration across clinicians and providers.

Qualifikationen

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with the ability to collaborate across disciplines.
• Comfortable working independently in a fast-paced, field-based environment.

Aufgaben

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
