VitalCaring Group in Webster, TX is seeking a Clinical Care Manager (RN) to lead coordination of home health care across the patient journey. You will oversee care plans, collaborate with clinicians and providers, and guide clinical decisions to ensure high-quality outcomes.

Qualificações

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Demonstrates strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with ability to collaborate across disciplines.

Responsabilidades

• Lead and coordinate care across the full patient journey within home health.
• Manage physician orders, documentation, and workflows in the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
