Job summary

Hope Network is seeking a Lead Direct Care Professional to provide oversight, support, and direct care across settings. You will lead on shift, mentor staff, and help implement medical, clinical, and behavioral plans to promote independence. The role emphasizes person-centered care and teamwork.

Qualifications include a Bachelor’s or higher with related experience, or an Associate or high school diploma with more years. Growth opportunities, paid training, and competitive benefits are offered.

Qualifications

• Bachelor's degree with 2 years experience in applicable setting.
• Associate's degree with 3 years experience in applicable setting.
• HS Diploma/GED with 4 years experience in applicable setting.
• Demonstrated critical thinking/problem solving skills to respond appropriately to medical and behavioral issues.
• Demonstrated leadership skills and ability to problem solve independently.

Responsibilities

• Provide leadership, direction and support to staff on shift in coordination with the supervisor.
• Schedule, training, compliance, on-call pager, and administrative supports.
• Elevate employee relations concerns to supervisor; assist on special projects.
• Input in the development of the consumer’s treatment plans and goals; ensure plans are implemented.
• Implement and participate in the clinical and behavioral goals for each consumer using approved methods.
• Provide accurate daily observational data and document per agency standards.
