Job summary

Rosecrance Health Network seeks a Team Leader for the Community Support Team at the Ware Center in Rockford. The role oversees clinical staff, manages caseloads, and ensures timely documentation in the EHR while maintaining quality care standards.

Preferred candidates hold a Master’s degree in a related field and QMHP eligibility per IL DMH Rule 132. The position is onsite with standard weekday hours and opportunities for professional growth and impact.

Qualifications

• Master's degree or equivalent experience in a related field.
• QMHP qualification per Illinois DMH Rule 132 preferred.
• Experience supervising a team in social services is a plus.
• Ability to motivate staff and solve problems.
• Ability to work with diverse backgrounds and communicate effectively.
• Proficient in documentation and EHR systems.

Responsibilities

• Provide supervision and daily oversight of the clinical team.
• Manage caseloads, run groups, and coordinate care.
• Complete timely documentation and required regulatory reports.
• Train staff, perform quality assurance, and attend meetings.
• Participate in on-call system and performance feedback cycles.
• Ensure confidentiality and professional conduct at all times.
• Deliver high-quality customer service and support to clients.
• Represent the agency in a positive, professional manner.
