Job summary

QueensCare is seeking a Lead Community Health Worker to supervise CHWs, coordinate scheduling, and deliver comprehensive community health coordination across multiple sites in Los Angeles County.

You will perform direct patient case management, assist with healthcare access, and lead a collaborative Health & Faith Partnership team, ensuring CHWs stay trained and compliant with program requirements.

Qualifications

• High School Diploma or GED required or equivalent.
• Bachelor's degree preferred in Public Health, Health Administration, Social Work or related field.
• CHW certification preferred.
• Experience working with diverse populations in health settings is desirable.

Responsibilities

• Supervise and coordinate daily work of Community Health Workers to ensure quality patient support.
• Schedule staff and manage timecards and coverage.
• Provide case management and healthcare access support to patients, including housing status.
• Conduct outreach and health education at community sites and health fairs.
• Ensure training and regulatory compliance for CHWs and staff development.
• Travel regularly to partner sites across Los Angeles County to deliver services.
