Job summary

NYC Health + Hospitals Queens Hospital Center seeks a Community Health Worker to support patients, families, and caregivers around developmental, behavioral, and health-related social needs. You will participate in TeamUp activities and collaborate with interdisciplinary teams to improve pediatric care integration from birth through adolescence.

Responsibilities include outreach, patient recruitment, coordination with Epic and other resources, and training CHWs.

Qualifications

• Three years of full-time experience in counseling, community work or community health activities.
• Substitution: a combination of education and/or experience equivalent to the above, with at least one year in community/public service work.

Responsibilities

• Partner with CHW lead and clinical leadership to deliver TeamUp CHW program.
• Recruit and hire CHW candidates as needed.
• Train and mentor CHWs with individualized learning plans.
