Job summary

Mercy is seeking a Supervisor of Integrated Health & Social Care in Rogers, AR. You will lead a team of Community Health Workers, helping patients navigate healthcare and community resources, and address social determinants of health.

You will coordinate care, support financial assistance access, and partner with Mercy leadership to improve outcomes while advocating for patients’ needs.

Qualifications

• Bachelor's degree in community health, public health, or related coursework.
• 3+ years CHW experience or equivalent.
• Certified in at least one chronic disease program (e.g., DPP) and Basic Life Support.
• Experience with Epic EMR, reporting, and dashboards.

Responsibilities

• Lead, coach, and develop a team of Community Health Workers.
• Support uninsured and underinsured patients in accessing healthcare and financial assistance.
• Connect patients with resources addressing social needs like transportation and food access.
• Help patients manage chronic conditions and achieve health goals.
• Build partnerships with community organizations and Mercy leaders to improve outcomes.
