Job summary

North Suffolk Mental Health Association is seeking a Clinical Coordinator to lead an Intensive Community Wrap-Around Services Team. The role involves supervising assessments, treatment planning, and service delivery in home and community settings, guiding a team as they implement trauma-informed, person-centered care.

The position emphasizes developing and reviewing treatment plans, coordinating with a multidisciplinary team, and supporting recovery-focused outcomes for adults with mental health

Qualifications

• LPHA licensure required: LICSW, LCSW, LMHC, LRC, LADC1, M.D., R.N., Registered Occupational Therapist or licensed Psychologist.
• Previous experience in clinical or rehabilitative services for adults with chronic mental health issues and/or substance use disorders preferred.
• Valid Driver's license; MAP certification and CPR/First Aid certification within 5 months of hire.

Responsibilities

• Engage people and partner with them to improve health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness and live a self-directed life.
• Assist people with autism spectrum disorder, psychiatric and substance use disorders to identify priorities and goals to support recovery.
• Provide clinical services in trauma informed, person-centered care consistent with principles and practices.
• Carry out treatment plan interventions with direct support and rehabilitation services in office and community settings to teach behavioral symptom-management techniques.
• Organize and lead individual and group social activities to structure time, increase social experiences, and practice social skills.
• Document progress in confidential records according to established methods.
• Other duties as assigned by the Program Director.
