Responsibilities

• Collaborate with interdisciplinary team to develop and educate care plans.
• Coordinate staffing and resources to meet patient needs safely and cost-effectively.
• Lead the care team, delegating and ensuring adherence to care plans and standards.
• Monitor outcomes, adjust plans, and communicate with patients and families.
• Support continuous quality improvement and enforce policies.
