Job summary

Saint John’s Program for Real Change in Sacramento seeks a Case Manager for a temporary position with potential for temp-to-hire. The role pays $22.00–$25.00 per hour, full-time, on-site at Jackson Rd., Sacramento, CA, with Monday–Friday hours.

You will lead clients through goal setting, develop individualized service plans, coordinate with outside agencies, and maintain progress notes while applying trauma-informed approaches.

Qualifications

• Social Work or Psychology Bachelor’s degree plus 1 year of experience; OR AA degree plus 2 years of experience working with individuals or families with complex needs.
• Works independently, flexibly, and as part of a team.
• Works effectively with outside agencies in providing client services.

Responsibilities

• Assess client and family needs and strengths; conduct thorough assessments.
• Develop, implement, and review individualized service plans with clients.
• Coordinate with community resources to meet client needs and objectives.
• Demonstrate proficient verbal and written communication, and conflict management.
• Maintain accurate progress notes for all clients on caseload; attend required staff meetings.
• Provide case management to clients and families, including behavior management techniques.
• Facilitate psychoeducation classes and groups.
• Handle on-duty assignments providing crisis intervention, resources, and referrals.
