Job summary

South Shore Health is seeking an experienced RN Case Manager to advocate for patients, coordinate care, and ensure appropriate utilization across the continuum. The role requires collaboration with physicians and payers to determine medical necessity and to support efficient discharge planning.

The position emphasizes quality improvement, cost-efficient care, and on-site availability seven days a week, including weekends and some holidays. A BSN and CCM/ACM within two years are preferred.

Qualifications

• Minimum education: RN; BSN preferred.
• 3-5 years acute care hospital experience preferred.
• Critical Care or Emergency Department experience highly desirable.
• ACM or CCM certification within two years of hire preferred.

Responsibilities

• Review medical records for observation/admission and ensure appropriate utilization.
• Coordinate discharge planning and care coordination with multidisciplinary teams.
• Collaborate with physicians, nursing, admitting and payors to determine patient status and necessary care.
• Facilitate timely discharge planning and transitions to lower levels of care.
