Responsibilities

  • Educate, assist, and advocate for families facing lead and environmental health hazards to help safe and stable housing.
  • Conduct in-home and virtual visits to educate tenants on rights, lead poisoning prevention, asthma, and health hazards.
  • Provide case management to increase housing stability and energy security, improving health and educational outcomes.
  • Assist relocation to lead-safe housing when needed and coordinate available resources.
  • Serve as a liaison between families and appropriate agencies for services and assistance.
  • Help eligible Baltimore City residents receive lead preference Housing Choice Vouchers.
  • Track clients and provide follow-up for voucher recipients.
  • Coordinate Relocation Assistance Grant Program including reviews and disbursement.
  • Conduct trainings and presentations to health and housing professionals and community partners.
  • Report data in ETO Client Database and maintain weekly project reporting.
  • Assist in monthly, quarterly, and annual reporting to funders.
  • Participate in team meetings and provide research and policy recommendations.
  • Engage in outreach and community education events as needed.

Position Overview

The Case Manager-Family Advocate works to ensure that tenants’ rights are protected while lead hazards and other defects are remediated in client homes. This position provides tenant’ rights assistance, case management, resource navigation, and lead poisoning prevention, asthma and healthy homes education statewide to reduce lead poisoning, asthma episodes and household injury. The Case Manager-Family Advocate conducts home visits with families in Baltimore City and throughout the Baltimore Metro region, as well as occasional visits elsewhere in the state. The position serves on the Maryland Programs Department team and assists the Vice President of Maryland Programs and the Director , Family Advocacy Services in the effective implementation of the Family Advocacy Services Program by recommending and assisting in the implementation of initiatives, policies and processes to improve client outcomes, program operations, enhance the client experience and promote program partnerships.

Duties and Responsibilities
  • Educate, assist, and advocate for families facing lead and other environmental health hazards in their homes in order to help those families achieve safe and stable housing including but not limited to: helping tenants to send written Notices of Defect to their rental property owner for the repair of hazards, coordinating enforcement actions with City and State agencies, and facilitating client resource referrals.
  • Conduct in-home and virtual visits to educate tenants on tenant’s rights, lead poisoning prevention, asthma, and other home-based environmental health hazards.
  • Provide case management support to client families to increase housing stability and energy security, and improve health, educational and social outcomes.
  • Where the best option for a lead-affected family is to relocate from the property, work with client families to help facilitate that move to lead safe housing and coordinate resources including possible relocation assistance grant applications where that funding exists.
  • Serve as a liaison between lead-affected families and appropriate agencies when seeking services and assistance.
  • Assist eligible Baltimore City resident families receive lead preference Housing Choice Vouchers to relocate from lead hazardous housing to lead certified housing.
  • Track clients and provide resources and follow-up assistance to Baltimore City Housing Choice Voucher Program recipients receiving lead preference vouchers.
  • Coordinate GHHI’s CDBG funded Relocation Assistance Grant Program including application review, purchase order processing, case management and funds disbursement.
  • Conduct Green & Healthy Homes trainings and presentations to health and housing professionals, parents, property owners, community residents, and community partners among others .
  • Report data in GHHI Efforts to Outcomes (ETO) Client Database in compliance with standards and protocols.
  • Maintain responsibility for internal weekly project reporting.
  • Assist the Vice President of Maryland Program Services and Director, Family Advocacy Services in preparing monthly, quarterly and annual reporting to funding agencies.
  • Participate in weekly and other team meetings as appropriate. Provide research, analysis and recommendations on policy and program best practices.
  • Participate in outreach and community education activities and events on an as-needed basis, including occasional events that support program visibility, education, and client engagement.
  • Other duties as assigned related to organizational mission and programs.

MINIMUM QUALIFICATIONS

  • Bachelor’s or Associate’s Degree in public health, environmental science, social work or related field.
  • Two years of experience in non-profit, public or private agency client case management or client services logistics coordination.

Position-Specific Requirements

This position will be assigned duties that require the operation of a motor vehicle. The incumbent must maintain ready access to a motor vehicle, possess a valid motor vehicle operator’s license in the State of Maryland, and required auto insurance.
