Potomac Case Management is seeking a LEAD Case Manager to coordinate harm reduction services, peer support, and community case management in Washington County, Maryland. You will collaborate with the Sheriff’s Department, Health Department, and Mobile Crisis to divert potential law enforcement intervention and maintain client engagement with services.

The role requires experience in behavioral health, strong communication skills, and adherence to program policies and confidentiality standards.

Qualificações

• Bachelor’s degree in Human Services preferred.
• Minimum 2-year experience in behavioral health working with adults.
• Experience coordinating and/or delivering services to clients with homelessness, substance abuse, and/or mental health issues.
• Strong interpersonal skills.
• Excellent verbal and written communication skills.

Responsabilidades

• Liaises with community agencies and other local mental health providers, promoting effective communication.
• Attends and participates in all scheduled monthly staff meetings and caseload reviews.
• Maintains confidentiality and HIPAA compliance in all interactions and records.
• Records time and completes documentation by 10am the next business day.
