Job summary

The Senior Case Manager at Baylor College of Medicine supports timely access to behavioral health services by maintaining an assigned caseload and coordinating the daily workflow of the case management team.

The role assigns and prioritizes cases, monitors workload and follow-up needs, guides and trains case managers, connects families with recommended resources, and maintains complete documentation in required clinical and state systems.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services or a related field.
• Four years of relevant experience.

Responsibilities

• Provides day-to-day guidance, support, onboarding assistance, and training to case managers on program workflows, resource navigation, documentation expectations, and complex service-coordination needs.
• Monitors case-management workflow and referral volume; assign and prioritize families, redistribute work when needed, track pending actions, and elevate access, capacity, or service-delivery concerns to program leadership.
• Collaborates with the multidisciplinary treatment team to identify appropriate behavioral health, social-service, school, and community resources for children and families.
• Manages an assigned caseload and assist families with connecting to recommended resources, including outreach, follow-up, barrier resolution, and confirmation of successful linkage to services.
• Represents the program at community events and develops working relationships with community organizations to expand knowledge of available resources and referral options.
• Maintains timely, accurate, and complete documentation in the electronic health record, state-required systems, and other program tracking platforms in accordance with program, institutional, and regulatory requirements.
• Performs other job-related duties as assigned.
