Job summary

The W Group is seeking a compassionate Lead Case Manager to oversee resident services and guide a team of case managers. You will coordinate admissions from community, nursing homes, and hospitals while ensuring high standards of care and documentation.

The ideal candidate has 2+ years in case management within long-term care, strong communication skills with families and care providers, and the ability to lead a multidisciplinary team. Bachelor’s degree is preferred but not required.

Qualifications

  • 2+ years' experience as a Case Manager in long-term care.
  • Prior case management and Compuware experience is a plus.
  • Excellent phone skills and ability to work with families and other care providers.
  • Strong organizational skills with compassion for residents.
  • Ability to lead and collaborate with a multidisciplinary team.
  • Bachelor's Degree preferred but not required.

Responsibilities

  • Maintain adherence to the resident's Bill of Rights within the facility.
  • Maintain meaningful communication with residents' care providers.
  • Lead assessments and coordinate admissions from community, nursing homes and hospitals.
  • Coordinate and supervise clinical care for residents.
  • Mentor and guide other case managers to ensure consistent processes.
  • Ensure clear documentation to facility standards.
  • Assist in training and onboarding new staff.

The W Assisted Living Management Company is seeking a compassionate and dedicated Lead Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with a positive and supportive team of healthcare professionals, while helping guide and mentor fellow case managers.

Job Responsibilities
  • Maintain firm adherence to the resident's "Bill of Rights," as they relate to the facility
  • Maintain meaningful communication with the resident's care providers
  • Lead assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
  • Coordinate and supervise clinical care for residents
  • Serve as a point of guidance for other case managers, helping ensure consistency in process and documentation
  • Clear and concise documentation in accordance with facility requirements
  • Assist in training and onboarding new case management staff
Qualifications/Requirements
  • 2+ years' experience as a Case Manager, ideally within Long Term Care
  • Previous case management and Compuware experience a plus
  • Excellent phone skills and ability to work with families/other care providers
  • Strong organizational skills, passion and compassion for the population
  • Ability to lead and work cooperatively with a multidisciplinary team
  • Bachelor's Degree preferred but not required
