Responsibilities include ensuring contract compliance, collaborating with multidisciplinary teams, and maintaining case records.

Qualifications

• Master's Degree in social work, Mental Health Counseling or related Human Services field.
• Two years of professional experience with homelessness or marginalized populations.
• Knowledge of case management, ethics and quality standards.

Responsibilities

• Supervise case management operations and staff in assigned area.
• Coordinate with referrals to legal, medical, housing and social services.
• Lead case conferences and trainings to build competencies.
• Review participant plans, ensure accuracy of records and reports.
• May carry a caseload as needed and ensure client outcomes.
