Qualifikationen

• Home Care Aide certification.
• At least two years of caregiving experience in long-term care.
• Current CPR/BLS certification.
• Full-time employment status.
• Meets initial leadership and continuing education leadership training requirements.
• Must have no disciplinary action in effect against certification.

Aufgaben

• Provides oversight and assistance to caregivers during shifts and ensures adherence to scheduled cares.
• Assists residents with ADLs and medication services per care plans and policies.
• Oversees schedules (shower, laundry, vital signs, etc.) to ensure daily adherence and documentation.
• Assigns tasks, distributes workload, and supports nurses and supervisors as needed.
• Maintains care documentation in the electronic medical record and reports changes to nurse.
• Supports resident transfers, feeding assistance, and dining room coverage when needed.
• Monitors resident condition, weight, and vitals per facility standards and reports changes.
• Attends care conferences and participates in care plan updates and QA activities.
