Saban Community Clinic is hiring a Lead Care Manager in Hollywood, CA. The role focuses on coordinating ECM services, implementing Care Plans, and connecting patients to essential social supports within MCP guidelines.

You will advocate for patients, utilize trauma-informed and harm-reduction approaches, and support discharge planning while monitoring adherence and promoting health self-management.

Qualifications

• Bachelor and/or Master’s degree in social work or related field or equivalent experience.
• 3 years prior case management experience.
• Valid California driver’s license and insurance.
• Related work experience in public health, health outreach and education.
• Fluency in another language such as Spanish, Russian or Korean preferred.

Responsabilités

• Coordinate with individuals and entities to ensure a seamless experience for the member and prevent duplication of services.
• Oversee provision of ECM services and implement Care Plan.
• Connect member to social services and support needs, such as transportation and housing.
• Advocate for members with health care professionals.
• Use motivational interviewing, trauma-informed care, and harm-reduction approaches.
• Coordinate with hospital staff on discharge plan.
• Accompany member to office visits as needed per MCP guidelines.
• Monitor treatment adherence including medication.
• Provide health promotion and self-management training.
• Inform patients about other services offered by Saban Community Clinic for family members.
