Job summary

Lifespan is seeking a dedicated Care Manager in Rochester, NY, to bridge APS with community aging services. You will assess needs, develop care plans, and connect clients with Lifespan programs and community resources.

This is a full-time, 37.5-hour role focused on coordination and client advocacy. Responsibilities include joint assessments with APS, leading care management activities, and coordinating referrals while maintaining up-to-date knowledge of geriatric services and entitlements.

Qualifications

• Education: Bachelor or Master of Social Work or equivalent degree.
• Experience: Two years in social work; at least one year in geriatrics.
• Bilingual preferred but not required.

Responsibilities

• Provide care management to clients.
• Accept referrals as assigned and complete home assessments.
• Develop client-centered care plans with APS and connect to services.
• Arrange mental health interventions and provide supportive counseling.
• Lead project activities and coordinate other care managers.
• Participate in team meetings, trainings and quality assurance.
