Job summary

Carle Health is seeking an Exercise Physiologist to monitor outpatient cardiac rehab and conduct non-invasive cardiovascular exams. The role collaborates with the care team to implement exercise programs for phase II and III rehab and to ensure post‑op care alignment.

You will evaluate patient progress, provide education, document in EMR, assist with scheduling and billing, and supervise students in the program as needed. On-site position in Illinois.

Qualifications

• BLS certification within 30 days
• Bachelor's Degree in a related field

Responsibilities

• Develops, implements and evaluates exercise programs for cardiac patients in phase II and III.
• Collaborates with RN and physician to ensure post-op and/or post-hospital care.
• Evaluates and revises plan of care as needed in collaboration with RN and physician.
• Develops and advances the patient's exercise program based on the physiological response to exercise.
• Provides health education and teaching for patient and family.
• Completes feedback and communication to patient and healthcare team including EMR documentation.
• Assists with scheduling and billing of cardiac rehab services.
• Precepts nursing, exercise physiology and nutrition students as assigned.
• Schedules patient for cardiac rehab initial visits.
• Conducts initial visit patient appointments.
• Performs patient heart disease risk factor assessments.
• Coordinates patient care plans for outpatient cardiac rehab, phase II and III.
• Coordinates patient exercise class schedules.
• Monitors patients on telemetry during exercise sessions.
• Leads patients through exercise prescriptions and assists with equipment.
• Provides education to patients to support risk factor modification.
• Reports adverse events to ordering providers.
• Monitors patients during exercise stress tests.
• Applies ambulatory holter and event monitors.
