Job summary

Catholic Health Home Care is seeking a Clinical Project Manager – Behavioral Health to supervise the Behavioral Health Home Care Program, guiding nursing staff and clinical operations to ensure safe, patient-centered care for individuals with psychiatric needs.

The role collaborates with hospitals, physicians, and families to coordinate discharge planning, quality improvement, and regulatory compliance, while supporting organizational planning and ongoing program development.

Qualifications

• Current RN license in NY required.
• Bachelor’s in Nursing; Master’s preferred.
• Psychiatric/Behavioral Health nursing experience required.
• Previous clinical leadership/management experience.
• Home care, discharge planning, or care coordination preferred.
• Strong knowledge of psychiatric nursing principles and regulatory requirements.
• Excellent communication, leadership, organization, and problem-solving skills.
• Ability to collaborate with interdisciplinary teams and healthcare partners.
• Demonstrated clinical judgment in complex patient care.
• Proficiency with electronic medical records and related systems.

Responsibilities

• Oversee daily operations of the Behavioral Health Home Care Program.
• Lead Behavioral Health Nursing staff and clinical teams.
• Coordinate safe and patient-centered care delivery.
• Facilitate discharge planning with hospitals and facilities.
• Provide clinical guidance to after-hours staff on psychiatric cases.
• Participate in organizational planning and program development.
• Ensure compliance with policies, laws, and accreditation standards.
• Promote service excellence in all interactions with patients and partners.
• Support staff development and ongoing competency assessments.
• Monitor clinical documentation and regulatory adherence.
