Job summary

Catholic Health is seeking an experienced Clinical Project Manager – Behavioral Health to oversee the Behavioral Health Home Care Program. You will lead nursing staff, coordinate care, and ensure safe, patient-centered transitions for psychiatric and behavioral health patients.

The role requires RN licensure in New York, a BSN (Master’s preferred), and solid leadership experience in home care or discharge planning. Strong regulatory knowledge and EMR proficiency are essential.

Qualifications

• Current RN license in New York state.
• Bachelor’s degree in Nursing; Master’s degree preferred.
• Psychiatric/Behavioral Health nursing experience required.
• Previous clinical leadership, supervisory, or management experience.
• Home care, community health, hospital discharge planning, or care coordination experience preferred.
• Strong knowledge of psychiatric nursing, behavioral health care, patient safety, and regulatory requirements.
• Excellent communication, leadership, organizational, and problem-solving skills.
• Ability to collaborate with interdisciplinary teams and healthcare system partners.
• Demonstrated clinical judgment in complex patient situations.
• Proficiency with electronic medical records and clinical/administrative information systems.

Responsibilities

• Manage daily operations of Behavioral Health Home Care Program.
• Provide leadership to Behavioral Health Nursing staff and other personnel.
• Coordinate and deliver safe, effective, patient-centered care.
• Liaise with hospitals and facilities to facilitate safe discharge planning.
• Provide clinical guidance to on-call staff for after-hours psychiatric/behavioral concerns.
• Participate in organizational planning, policy development, and program development.
• Ensure compliance with policies, laws, regulations, and accreditation standards.
• Promote service excellence in interactions with patients, families, staff, and partners.
• Provide clinical consultation to staff on psychiatric/behavioral health concerns.
• Participate in orientation, competency assessments, and ongoing professional development.
• Monitor timely completion and submission of clinical documentation.
• Ensure adherence to documentation requirements and regulatory standards.
