Job summary

LifeLong Medical Care operates Arnold's Place in Alameda, a 50-bed residential setting offering onsite medical and behavioral health services for unhoused patients recovering from procedures or hospitalizations. The Behavioral Health Provider will work with an inter-disciplinary team to deliver psychosocial services, group activities, and individual therapy in a supportive milieu.

The role reports to the Associate Behavioral Health Director and requires CA LCSW/LMFT/LPCC licensure, collaboration

Qualifications

• Ability to work effectively and collaboratively in a fast-paced primary health care setting.
• Must be a self-starter able to balance counseling, case management and administrative tasks.
• Demonstrated ability and sensitivity to working with a diverse staff and patient population.
• Excellent verbal, written and interpersonal communication skills; strong organizational and problem‑solving skills.
• Ability to supervise staff and contribute to staff development.

Responsibilities

• Provides comprehensive psychosocial assessments and short-term counseling for patients with depression, anxiety, trauma, grief, addiction, and behavioral health needs.
• Provides short-term case management services for patients in treatment.
• Assists with crisis intervention and triage as needed, including de-escalation and coordination with crisis response teams.
• Maintains clinical and administrative records and submits encounter forms weekly.
• Participates in weekly clinical meetings to promote integrated primary/mental health care.
