Job summary

LifeLong Medical Care seeks an LCSW/LPCC to join the Supported Housing site at Dellums Apartments in Oakland, CA. Under supervision, you will deliver psychosocial services to patients and work within an inter-disciplinary team to coordinate care.

You will perform assessments, develop treatment plans, and provide short-term counseling while supporting crisis intervention, case management, and follow-up planning in a primary health care setting.

Qualifications

• Current and valid California LCSW or LPCC license.
• Experience providing mental health services to disadvantaged populations.
• Familiarity with evidence-based practices for depression and other mental disorders.

Responsibilities

• Provide psychosocial assessments, diagnoses, treatment plans, and short-term counseling.
• Provide short-term case management and crisis intervention as part of a multidisciplinary team.
• Maintain clinical and administrative records per contracts and billing requirements.
