South Central Primary Care Inc. is seeking a Licensed Clinical Social Worker (LCSW) in Georgia to counsel clients, assess psychosocial needs, and connect patients with necessary resources.

The role requires collaboration with MDs, NPs, and PAs, and involves developing treatment plans and maintaining EMR documentation. The ideal candidate will hold a Master’s Degree in Social Work, maintain a Georgia LCSW license, and possess strong communication, problem-solving, and crisis intervention skills.

Formación

• Must hold a Master’s Degree in Social Work from an accredited university/college.
• Maintains Georgia state license to practice clinical social work.
• Maintains current CPR certificate.
• Demonstrates knowledge of mental health care concepts and medical case management.

Responsabilidades

• Conducts thorough psychosocial assessments and develops treatment plans.
• Advocates for patients and coordinates with healthcare providers and community resources.
• Documents accurately in electronic medical records and maintains confidentiality.
• Provides health education and crisis intervention as needed.
• Participates in staff meetings and departmental functions and travels to facilities.
